This form is for requesting a temporary hold on your recurring monthly membership, not for cancellation.
You may place your membership on hold for up to 3 months per calendar year. Additional hold time may be considered under special circumstances.
Submission Deadline: Please submit your request at least 15 days before your next billing cycle. Reactivation: Your membership will automatically reactivate at the end of the hold period, and regular billing will resume.
